What is Form E111?
This is the Community document which demonstrates that the holder is entitled to free or reduced cost emergency medical treatment. So if you or your dependants are suddenly taken ill or have an accident during a visit to an EEA country, you will receive treatment on the same terms as nationals of the country you are visiting when you produce a valid E111.

Who is eligible for Form E111?
You are eligible to obtain Form E111 if you are resident in the UK and you are: a national of the UK; a national of any other EEA country; a stateless person or refugee; a widow receiving a UK state pension or Widows Benefit whose late husband was a national of an EEA country, living in the UK at the time of his death. Please note: your E111 will cease to be valid if you go to live outside the UK.

How do I obtain Form E111?
An application for Form E111, as well as the form itself, is inside the Health Advice for Travellers booklet, available over the counter at main post offices. You need to complete the details on the application form and the E111 and take them to be signed and stamped at the post office. Form E111 is free - you will not have to pay for its issue. You should then keep it safe, preferably with your passport, and carry it with you when travelling within the EEA. REMEMBER - You must get your E111 stamped and signed by the post office for it to be valid.

Is there a time limit on the E111?
Your E111 remains valid indefinitely as long as you remain ordinarily resident in the UK. You will only need to apply for another E111 if you use it to claim for treatment and it is not given back to you, or if you mislay it.
